Salucro may amend this policy at any time by posting a revised version on our website. The revised version will be effective at the time posted. In addition, if the revised version includes a substantial change, Salucro will provide you with 30 days’ prior notice by posting notice of the change on the “Policy Updates” page of our website.
The use of information collected through our service is limited to the purpose of providing the service for which the Client has engaged Salucro Healthcare Solutions.
BINDING CORPORATE RULES
HOW SALUCRO COLLECTS INFORMATION ABOUT YOU
When you visit the Patientportal.me website or use the Patientportal.me Services, Salucro collects your IP address and standard web log information, such as your browser type and the pages you accessed on Salucro.com. If you open a patient account or use the Patientportal.me Services, Salucro may collect the following types of information:
- Contact information – your name, address, phone, email, and other similar information.
- Financial information – the full bank account numbers and/or credit card numbers that you link to complete payment, or provide to Salucro when you use Patientportal.me Services. This information is then deleted, except in the case of a recurring payment.
Before permitting you to use the Patientportal.me Services, Salucro may require you to provide additional information to verify your identity or address or manage risk, such as your healthcare provider account number, date of birth, social security number or other information. Salucro may also obtain information about you from third parties such as healthcare providers, credit bureaus, and identity verification services to help us with customer authentication and credit-related decisions.
When you are using the Patientportal.me Services, Salucro collects information about your transactions and your other activities on the company website or using the Patientportal.me Services and may collect information about your computer or other access device, but only for fraud prevention purposes.
Finally, Salucro may collect additional information from or about you in other ways such as contacts with the Salucro customer support team or results when you respond to a survey from interactions with other products of Salucro Healthcare Solutions, LLC.
When you access Salucro.com, Salucro, or companies Salucro hires to track how our website is used, may place small data files called “cookies” on your computer. Salucro sends a “session cookie” to your computer when you log in to your account or otherwise use the Patientportal.me Services. This type of cookie helps Salucro to recognize you if you visit multiple pages on the site during the same session; so that you are not prompted for a password on each page. Once you log out or close your browser, this cookie expires and no longer has any effect.
Salucro also uses persistent cookies or longer-lasting cookies for other purposes such as to display your e-mail address on our sign-in form, so that you are not required to manually re-enter the e-mail address each time you log in to your account. A persistent cookie remains on your hard drive for an extended period of time.
Salucro encodes our cookies so that only Salucro can interpret the information stored in them. You are free to decline our cookies if your browser permits, but doing so may interfere with your use of the company website.
As is true of most web sites, Salucro gathers certain information automatically and stores it in log files. This information may include internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and clickstream data.
Salucro uses this information, which does not identify individual users, to analyze trends, to administer the site, to track users’ movements around the site and to gather demographic information about the company’s user base as a whole. Salucro does not link this auto-collected data to personal information. IP addresses are tied to personal information to increase convenience of use and security onsite.
Salucro’s third party partners employ clear gifs (a.k.a. Web Beacons/Web Bugs), images, and scripts that help them better manage content on our site. Salucro does not tie the information gathered to personal user information.
HOW SALUCRO PROTECTS AND STORES PERSONAL INFORMATION
Throughout this policy, Salucro uses the term “personal information” to describe information that can be associated with a specific person and can be used to identify that person. Salucro does not consider personal information to include information that has been anonymized so that it does not identify a specific user.
Salucro stores and processes your personal information on company computers and hosts computers in the US, and Salucro protects it by maintaining physical, electronic and procedural safeguards in compliance with applicable US federal and state regulations. Salucro uses computer safeguards such as firewalls, data encryption when you enter sensitive information (such as credit card number), and vulnerability scanning.
In addition, Salucro enforces physical access controls to company buildings and files, and authorizes access to personal information only for those employees who require it to fulfill their job responsibilities. However, method of transmission over the Internet, or method of electronic storage is not 100% secure. Therefore, Salucro cannot fully guarantee its absolute security. HOW SALUCRO USES THE PERSONAL INFORMATION WE COLLECT
Salucro’s primary purpose in collecting personal information is to provide you with a secure, smooth, efficient, and customized experience. Salucro may use your personal information to:
- Provide the Patientportal.me Services and customer support you request;
- Process transactions and send notices about your transactions;
- Resolve disputes, collect fees, and troubleshoot problems;
- Prevent potentially prohibited or illegal activities, and enforce the company User Agreement;
- Customize, measure, and improve the Patientportal.me Services and the content and layout of our website and applications;
- Deliver service update notices;
- Compare information for accuracy and verify it with third parties, including your medical provider and the vendors they employ.
Salucro does not sell or rent your personal information to third parties for their marketing purposes. Salucro may combine your information with information collected from other companies and use it to improve and personalize Patientportal.me Services, content and advertising.
HOW WE SHARE PERSONAL INFORMATION WITH OTHER PATIENTPORTAL.ME USERS
To process your payments, Salucro may share some of your personal information with the healthcare provider that you are paying or that is paying you. Your contact information, date of sign-up, and the number of payments you have made to verifiedPatientportal.me customers. In addition, this and other information may also be shared with third parties when you use these third parties to access the Patientportal.me Services. Unless you have agreed to it, these third parties are not allowed to use this information for any purpose other than to enable the Patientportal.me Services.
If you are paying for a service through Patientportal.me, Salucro may also provide the healthcare provider with your confirmed credit card billing address to assist in completion of your transaction. If an attempt to pay your healthcare provider fails, or is later invalidated, Salucro may also provide your provider with details of the unsuccessful payment.
Salucro works with third parties, including healthcare providers, to enable them to accept or facilitate payments from or to you using Patientportal.me. In doing so, a third party may share information about you with us, such as your email address or mobile phone number to inform you when you attempt to pay a provider through that third party. Salucro uses this information to confirm that you are a Patientportal.me customer and that PayPal, a credit card, a debit card, a HSA card, a FSA card, or your checking account information as a form of payment can be enabled. Also, if you request that Salucro validates your status as a Patientportal.me customer with a third party, the company will do so. Please note that healthcare providers you buy services from and contract with have their own privacy policies, and Salucro and Patientportal.me may not be held responsible for their operations, including, but not limited to, their information practices.
Regardless, Salucro will not disclose your credit card number or bank account number to anyone you have paid or who has paid you through Patientportal.me or with the third parties that offer or use the Patientportal.me Services, except with your express permission or if Salucro is required to do so to comply with credit card rules, a subpoena or other legal process.
HOW SALUCRO SHARES PERSONAL INFORMATION WITH OTHER PARTIES
Salucro may share your personal information with:
- Affiliated processors and subcontractors to provide joint content, products and services (like transactions and customer support), to help detect and prevent potentially illegal acts and violations of our policies, and to guide decisions about their products, services and communications.
- Service providers under contract who help with parts of our business operations such as fraud prevention, bill collection, and technology services. Salucro’s contracts dictate that these service providers only use your information in connection with the services they perform for us and not for their own benefit.
- Credit bureaus to report account information, as permitted by law.
- Law enforcement, government officials, or other third parties when
- Salucro is compelled to do so by a subpoena, court order or similar legal procedure
- Salucro needs to do so to comply with law or credit card rules
- Salucro and its business members believe in good faith that the disclosure of personal information is necessary to prevent physical harm or financial loss, to report suspected illegal activity, or to investigate violations of our User Agreement.
- Other third parties with your consent or direction to do so.
Patientportal.me will not sell or rent any of your personal information to third parties for their marketing purposes and will only share your personal information with third parties as described in this policy.
If you open a Patientportal.me account directly on a third party website or via a third party application, any information that you enter on that website or application (and not directly on a Patientportal.me website) will be shared with the owner of the third party website or application and your information may be subject to their privacy policies.
SERVICE PROVIDER, SUB-PROCESSORS/ONWARD TRANSFER
Salucro Healthcare Solutions may transfer personal information to companies that help provide our service. Transfers to subsequent third parties are covered by the provisions in this Policy regarding notice and choice and the service agreements with Salucro Clients. Salucro utilizes sub-processors in order to authorize payments and send email notifications or successful payment notifications.
Salucro Healthcare Solutions collects information under the direction of its Clients, and has no direct relationship with the individuals whose personal data it processes.
HOW YOU CAN ACCESS OR CHANGE YOUR PERSONAL INFORMATION
If you have a Patientportal.me login, you may review and edit your personal information at any time by logging in to your account and clicking the Settings tab.
Salucro will retain your information for as long as your account is active or as needed to provide your services. If you wish to cancel your account or request that Salucro no longer uses your information to provide you services, contact us at firstname.lastname@example.org. Salucro will retain and use your information as necessary to comply with the company’s legal obligations, resolve disputes, and enforce Salucro’s agreements.
Salucro may use your name and email address to send you marketing emails about products, which may be of interest to you. Out of respect for your privacy, you may choose to opt out of these marketing emails by following the unsubscribe instructions included in these emails, accessing the email preferences in your account settings page or you can contact Customer Support at email@example.com.
ACCESS TO DATA CONTROLLED BY SALUCRO’S CLIENTS
Salucro Healthcare Solutions has no direct relationship with the individuals whose personal data it processes. An individual who seeks access, or who seeks to correct, amend, or delete inaccurate data should direct his or her query to the Salucro Healthcare Solutions’ Client (the healthcare provider). If the Client requests Salucro Healthcare Solutions to remove the data, Salucro will respond to their request within 30 business days.
DATA RETENTION FOR DATA CONTROLLED BY OUR CLIENTS
Salucro Healthcare Solutions will retain personal data the company processes on behalf of Clients for as long as needed to provide services. Salucro Healthcare Solutions will retain and use this personal information as necessary to comply with legal obligations, resolve disputes, and enforce our agreements.
